Thomas Butler, un capitaine retraité de la Navy atteint de troubles cognitifs, passe des tests à l'hôpital lorsqu'il prend un otage en s'exprimant dans une langue étrangère avant de reprendre ses esprits. Le NCIS collabore avec l'agent Conrad de la CIA et apprend que l'homme était un espion russe dans les années 70, surveillé tout au long de sa vie. Quand un aide-soignant est tué et que Butler disparaît, le NCIS doit déterminer s'il représente une menace pour la sécurité nationale
